how far down the supply chain do we need to check, ie how do you define end user / consignee who fills the End User Undertaking?

Answer

The consignee is the entity in the foreign country that you deliver the goods to, and while they may also be end user they could be accepting the goods on behalf of someone else.

The end user is generally consider to be the person that uses the goods or incorporates them into a higher level assembly whereby the exported item loses its identity.
