Advice for customs and duties payable and taxes to be paid in US government?

Question

We are exporting hearing aids and cell phone devices to the USA from the UK.

Can you advise on all necessary customs and duties payable and any taxes that need to be paid within the US government. Also is there any form of declaration or reporting we need to submit to the US government with regards to the exports in terms of revenue generated to the US and any taxes or limitations once revenue increases?

We ship directly to the consumer from the UK and pay for shipping fees. They are small items and value rarely exceeds $1000.00 per consumer. I do believe that we invoice exclusive of VAT as there is no actual presence in the USA.
